Part-Time Position
Direct Care Professional (PT)
No previous experience required; we will provide paid training
Applicants must be age 18 or older
Employees are required to pass background study, have an operating vehicle, current driver's license and auto insurance (due to transporting clients)
Schedules built around applicants' availability and client needs
Weekend differential at 24-hour client sites for Sat/Sun day shifts
Sleep overnight stipends
Employees scheduled weekends work every other weekend
Full-Time Position
Case Manager (FT)
Require's a Bachelor's Degree (B.A., B.S.) in a human services field
One year of direct care work experience
Employees are required to pass a background study, provide evidence of degree in related area, and have an operating vehicle, current driver's license and auto insurance (due to transporting clients)
Will be responsible for conducting Direct Care Professional's 90-day performance reviews upon completion of orientation period.
Will be responsible for conducting site reviews as directed by the Services Director
Will be responsible for performing all of the responsibilities of the Direct Care Professional position as necessary (including direct client supervision)
Will be responsible for directly supervising Direct Care Professional including, but not limited to: directing work, conducting performance reviews, rewarding and disciplining employees, addressing complaints, and resolving conflicts
Will be responsible for processing staff Time Sheets and Expense Reports
Will be responsible for positive interactions with clients, guardians, client families, representatives of other agencies, and other persons with whom clients interact
Will be responsible for admission, orientation, and a 45-day review of new clients
Will be responsible for the development and maintenance of the Individual Program Plan (IPP) including, but not limited to: goal/objective writing, observation of program implementation, ensuring proper documentation, and service recommendations to the IDT
Will be responsible for the completing and distributing of Rule 40 Quarterly Reports and maintaining current Rule 40 Consent forms if applicable
Will be responsible for preparation of client transfer and discharge summaries
Will be responsible for compliance with Minnesota Rule 201 (Foster Care Licensing) and keeping client Foster Care Licenses current for each site
Will be responsible for the effective implementation of behavioral plans if applicable programming
Will be responsible for properly acting upon, documenting, and distributing Incident Reports
Will be responsible for the development and maintenance of the Risk Management Plan for each client
Will be responsible for the oversight, coordination, and proper documentation of all client health, medical, and psychiatric care
Will be responsible for balancing client financial accounts and maintaining accurate financial records for each client
Will be responsible for performing On-Call services when assigned
Other duties, as assigned
Supports and participates in common teamwork; cooperates and works together with supervisors and co-workers
Uses tactful, appropriate, and professional communication
